Identity card
The referral letter from your family doctor or specialist. This contains important information about your medication, early surgery and disease history. In addition, it may explain your current back problem.
The recent ( not older than six month )MRI, CT scans and X-ray’s are always reviewed by Dr. Van Daele himself. Please ask for the (internet)codes of the images at the hospital where these examinations took place and bring them with you. This way the images can be retrieved online. Please bring a printed copy of the radiology reports. This information is not always available online with the images.
The report of the nerve tests performed by a physiotherapist or neurologist (EMG, SSEP, MEP).
Do I need a referral letter for a consultation?
A referral letter is not necessary. You can make an appointment directly yourself. However, the information of a good referral letter can be of great informative value. Ask your GP, referring physician or specialist for this document.
IMPORTANT: A VISIT TO DR. VAN DAELE without a RECENT scan is of little use.
What are the waiting times for a consultation?
If you can agree on a flexible schedule, we have an average waiting time of 2 to 3 weeks.
Consultations by visiting emergency department, to reduce waiting time, are not possible. If an urgent appointment is necessary, your general practitioner or specialist can contact Dr. Van Daele directly to arrange this.
For a (real) urgency, you can always contact the emergency department 24/24.
Where do I sign up for a consultation ?
If you are coming for the first time to the O.L.V. of Lourdes hospital you have to register TWO times. That is why we ask you to arrive half an hour earlier than your first visit to our hospital.
1/ Only at the first consultation in Waregem: you will go to the DIENST INSCHRIJVINGEN. This desk is located at the entrance of the hospital. Here a "hospital number" is created, necessary to exchange information inside and outside the hospital.
2/ Everyone: You will go to the Orthopedics secretary's office . Here you will be registered and referred to waiting room nr 20 of Dr. Van Daele. You may already be given a questionnaire to fill in in the waiting room. This will be used to estimate the severity of your pain complaints.
How do I find the O.L.V. of Lourdes Hospital ?

The visitor parking is behind the hospital. You have to pay for the parking. Along the Vijfseweg it is forbidden to park your car.
